Linkage to Care

Continuity of care is central to our approach. We partner with local primary health care facilities and tertiary hospitals to ensure that clients with abnormal results are supported through follow-up and referral pathways.

How we support follow-up

  • We follow up results and contact clients with abnormal findings
  • Telephonic follow-up and, where necessary, home visits
  • Booking at the appropriate next-level facility for required procedures
  • Follow-up to confirm appointment attendance and outcomes
  • Transport support for those in need, depending on available funds

A structured referral pathway

We align referrals with Department of Health guidelines and work with partners to support timely access to care. Our model is designed to strengthen coordination between community services, schools, primary health care facilities, and hospitals.

Palliative Care

We support the introduction of palliative care from the time of diagnosis for people living with chronic illness. Clients diagnosed with cancer are referred and linked to palliative care services in collaboration with partners, including Chris Hani Baragwanath Academic Hospital.
