This training empowers community health workers and faith-based leaders to deliver cancer prevention and early detection education, using a replicable community model that covers lung, cervix, breast, prostate and multiple myeloma.
Continuity of care is central to our approach. We partner with local primary health care facilities and tertiary hospitals to ensure that clients with abnormal results are supported through follow-up and referral pathways.
We align referrals with Department of Health guidelines and work with partners to support timely access to care. Our model is designed to strengthen coordination between community services, schools, primary health care facilities, and hospitals.
We support the introduction of palliative care from the time of diagnosis for people living with chronic illness. Clients diagnosed with cancer are referred and linked to palliative care services in collaboration with partners, including Chris Hani Baragwanath Academic Hospital.
We provide supportive counselling and community-based follow-up to strengthen wellbeing for clients and families.
